What to send for your review
- A short description of your symptoms and how long you have had them
- Any recent test results (blood tests, PSA, hormone levels, semen analysis)
- Imaging or specialist reports (e.g. Doppler ultrasound)
- Current medications and relevant medical history
You can stay anonymous at this stage. A first name or alias is enough.
